By signing this form, I acknowledge, understand, and agree to the following:

1. Assumption of Risk

I understand that participation in soccer activities, including league matches, pickup games, training sessions, and related events organized by OC7SOCCER.COM LLC, involves inherent risks. These risks include, but are not limited to:

• Physical injury (minor, serious, or catastrophic)

• Collisions with players, referees, or objects

• Falls, slips, or contact with the playing surface

• Weather-related conditions

• Equipment-related injuries

I voluntarily choose to participate and assume all risks, known and unknown, associated with these activities.

2. Release of Liability

I hereby release, waive, and discharge OC7SOCCER.COM LLC, its owners, organizers, staff, referees, volunteers, sponsors, and affiliates, as well as the Huntington Beach Sports Complex, from any and all liability, claims, demands, or causes of action arising out of or related to:

• Any injury, illness, disability, or death

• Loss or damage to personal property

• Any incident occurring before, during, or after participation

I agree that I participate at my own risk.

3. Responsibility & Conduct

I agree to:

• Follow all league rules, guidelines, and referee decisions

• Respect other players, referees, staff, and spectators

• Avoid dangerous or unsportsmanlike behavior

• Accept that no slide tackling, violent conduct, or reckless play is allowed

I understand that failure to comply may result in removal from games, suspension, or expulsion without refund.

4. Medical Responsibility

I confirm that:

• I am physically fit and able to participate in soccer activities

• I have no medical condition that would prevent safe participation

• I am responsible for my own medical expenses in the event of injury

OC7SOCCER.COM LLC does not provide medical insurance coverage.

5. Media Release & Content Usage

I grant OC7SOCCER.COM LLC the right to:

• Record, film, photograph, and livestream my participation

• Use my image, likeness, voice, and name for promotional, marketing, and social media purposes

• Publish content on platforms including (but not limited to) Facebook, Instagram, YouTube, and websites

I understand that:

• No compensation will be provided for media usage

• All content is owned by OC7SOCCER.COM LLC

6. Payment & Refund Policy

I acknowledge that:

• All registration fees, league dues, and pickup payments are non-refundable

• No refunds will be issued for injuries, missed games, suspensions, or personal scheduling conflicts

7. Acknowledgment & Agreement

By signing below, I confirm that:

• I have read and fully understand this agreement

• I voluntarily agree to all terms stated above

• I understand that this is a legally binding waiver of liability

By signing up and confirming attendance (RSVP) to participate in soccer activities at Huntington Beach Sports Complex, you acknowledge that you are voluntarily participating and doing so entirely at your own risk.

You understand that soccer is a physical sport and involves inherent risks, including but not limited to serious physical injury, and you accept full responsibility for any risks that may arise from your participation.

In consideration of being allowed to participate, you agree to release and hold harmless OC7SOCCER.COM LLC, Bóng Đá Phủi Orange County (Bolsa Soccer), Phủi Bên Mỹ, Huntington Beach Sports Complex, and all associated organizers, including Hinh Phan (Organizer), along with their staff, volunteers, agents, and affiliates, from any and all claims, liabilities, damages, losses, or expenses (including legal fees) arising from your participation.

By participating, you agree not to bring any legal claims or lawsuits against the above parties for any injuries, damages, or losses that may occur during or as a result of these activities.

This agreement is binding upon you, your heirs, legal representatives, and assigns.

You confirm that you are 18 years of age or older, and that you have read, understood, and voluntarily agree to this waiver. By signing up and participating, you acknowledge that you are waiving certain legal rights and remedies.

If the participant is under 18 years of age, a parent or legal guardian must agree to this waiver on their behalf.
