By signing up and confirming attendance (RSVP) to participate in soccer activities in Huntington Beach Sports Complex, you acknowledge and agree that you are voluntarily participating in these activities and are doing so entirely at your own risk. You are aware that participation in soccer involves inherent risks, including but not limited to physical injury, and you assume full responsibility for any and all risks involved.

In consideration of being allowed to participate in soccer activities, you hereby release and hold harmless the Facebook group Bóng Đá Phủi Bolsa, Bolsasoccer.com (hereinafter referred to as "Bolsa Soccer") and Huntington Beach Sport Complex, including its staff, volunteers, agents, and any other individuals or entities associated with Bolsa Soccer and Huntington Beach Sport Complex, from any and all actions, claims, demands, liabilities, losses, costs, and expenses (including attorney’s fees) for any injury, loss, disease, or damage that you may suffer as a result of your participation in these soccer activities. This release extends to any and all claims you have or may have against Bolsa Soccer and Huntington Beach Sport Complex, whether known or unknown, arising directly or indirectly from your participation in soccer activities.

By participating, you agree not to sue Bolsa Soccer, Bóng Đá Phủi Bolsa, Huntington Beach Sport Complex or any of its affiliates for any injuries or damages you might experience during your participation in soccer activities. You agree that this consent is binding upon you, your heirs, executors, legal representatives, and assigns.

You affirm that you are of legal age and are freely signing this agreement. You have read this form and fully understand that by signing up and participating, you are giving up legal rights and/or remedies which may be available to you.
